HELPING PATIENTS MANAGE hyperphosphatemia is one of the biggest challenges facing renal dietitians. Current National Kidney Foundation Kidney Disease Outcomes Quality Initiative guidelines recommend dietary phosphorus restrictions between 800 and 1000 mg/day for patients with chronic kidney disease stages 3 to 5 who have elevated serum phosphorus levels.1 Practice guidelines also suggest that patients unable to control phosphorus levels with diet alone be placed on a phosphate binder, with the initial prescription tailored to the patient’s phosphorus intake.1 Although general guidelines exist for estimating the phosphorus content of foods, the exact phosphorus content of many foods is unknown because food labels are not required to list phosphorus under current regulations by the Food and Drug Administration (FDA) of the Department of Health and Human Services.
When a motivated patient inquires as to the phosphorus content of a favorite food, it is often a tedious, if not futile, process for the dietitian to obtain this information. In this case, a patient in my dialysis center requested information on Little Debbie Snack Cakes.2 My first step was to consult the Bowes & Church Food Values of Portions Commonly Used,3 which did not contain the information. Second, I checked with the United States Department of Agriculture or United States Department of Agriculture National Nutrient Database to no avail.4 Third, I performed a general Internet search for Little Debbie Snack foods, which yielded a company website. Unfortunately, the website did not list any nutrition information for their products. Under the “Contact Us” tab, I was able to send an e-mail request to the company for this information. They responded that the website could not provide this data but referred me to contact the customer service department and provided the phone number. After contacting customer service, I was told they had the basic nutrition information but not phosphorus content of their products. I asked if I could speak with the person responsible for their nutrition analysis and was given the voicemail for the employee in charge of school foodservice. After leaving a message for this individual along with my phone number and e-mail address, I received the material listed in Table 1, Table 2, Table 3, Table 4, Table 5, Table 6, Table 7, Table 8, Table 9, Table 10, Table 11, Table 12, Table 13, Table 14, Table 15.

Although this experience with the Little Debbie Company did produce the desired information, other such endeavors with different companies have not been successful. In addition, the Little Debbie Company was responsive in granting permission to publish this material within 1 week of receiving my request. Typically this process may take as long as 3 months (Lisa Murphy-Gutekunst, MSEd, RD, CSR, CDN, personal communication, 2006). Because nutrient listings for brand name foods are so difficult to obtain, a colleague suggested disseminating the Little Debbie information through this “Product Update” section of the Journal of Renal Nutrition for access by other nephrology professionals.
Currently, an estimated 8,300,000 people in the United States are affected by stage 3, 4, or 5 chronic kidney disease.1, 5 It would be of great benefit to these patients if the FDA would add the phosphorus content to food labels or at least require food manufacturers to provide such information on request. Although an FDA spokesperson reported no plans at the present time for changing the current food label, a citizen petition has been submitted to the FDA by a joint panel of the National Kidney Foundation/Council on Renal Nutrition and the American Dietetic Association Renal Practice Group.6 This petition has requested the inclusion of phosphorus, as well as potassium and calcium, in the nutrition fact panel on packaged foods. Unless the FDA approves this petition, obtaining such nutrition information will continue to be a “hit-or-miss” effort on the part of individual dietitians and the sharing of this material through publications such as the Journal of Renal Nutrition will remain an indispensable tool for helping patients manage hyperphosphatemia.
